We Provide

Skilled Nursing (RN/LVN)

  • Educates patients and families on medication, disease and pain management.
  • Manages chronic and acute conditions like heart failure, COPD and diabetes.
  • Provides wound care, including VAC, ostomy, and ileostomy support.
  • Administers infusion therapy, including IV antibiotics and TPN.

Physical Therapy (PT)

  • Improves mobility, strength, balance, and safety
  • Develops personalized home exercise programs
  • Supports post-surgical recovery (e.g., joint replacements)
  • Trains patients on safe use of assistive devices
  • Promotes independence and safe movement at home

Occupational Therapy (OT)

  • Helps regain independence in daily tasks (e.g., bathing, dressing)
  • Assesses and adapts the home for safety
  • Teaches use of adaptive equipment (e.g., grab bars, reachers)
  • Supports challenges related to cognition, vision, or physical function

Speech Therapy (ST)

  • Assists patients with speech, language, and swallowing difficulties.
  • Develops personalized strategies for memory, problem-solving, and cognitive function.
  • Provides swallowing therapy, including diet modifications for safe eating.

Medical Social Work (MSW)

  • Connects patients to community resources and support services.
  • Assists with long-term care planning, advance directives, and financial resources.
  • Provides emotional support and guidance for patients and families.

Home Health Aide (HHA)

  • Helps with daily tasks, including bathing, dressing, and grooming.
  • Provides companionship and emotional support.
  • Monitors patient conditions and reports changes to the care team.

President / Director of Patient Care Services (DPCS)

Mercy Wey is registered nurse (RN) with more than 30 years of clinical practice and an experienced healthcare administrator with a strong background in community health, nursing research, strategic planning, and healthcare administration and management. She holds a Doctor of Philosophy (PhD) in Nursing from the University of California, San Francisco.

     Lilian Lai

Rehabilitation

With nearly 20 years of experience, Lilian blends clinical expertise with a passion for leadership and mentorship. She holds a Master’s degree from the University of Southern California (USC) and a Doctor of Physical Therapy from A.T. Still University, bringing a well-rounded, culturally sensitive approach to patient care and team development.
