  • Medical Necessity – A physician determines that skilled nursing or rehabilitation services are required.
  • Physician Oversight – Patients must remain under a doctor’s care while receiving home health services. 
  • Referral & Evaluation – A physician refers the patient for home health services. Our team assesses the patient’s needs and eligibility.
  • Care Plan Development – A customized care plan is created based on medical conditions and goals.
  • Service Delivery – Skilled nurses, therapists, or aides provide care in the patient’s home.
  • Ongoing Monitoring – The care team tracks progress, adjusts treatment as needed, and communicates with the physician.
  • Discharge & Transition – When goals are met, the patient transitions to self-care or outpatient support
